The pretest is taken before participating in the SUSI program at your school.
The posttest is taken after participating in the SUSI program.

Please complete each of the following evaluation activities for each SUSI school.
Note: All of the links included below are password protected and should only be used by the YDCs. If you do not have the password, please contact the program director at GA-DPH.
Submit the First Progress Report.
This report addresses your efforts to form partnerships and plans for the implementation of SUSI at the participating schools/SUSI sites

Submit the Second Progress Report(s).
This report addresses your efforts to implement SUSI in each of your participating schools. (Note: If you are working with multiple schools, you will need to submit a separate form for each school).
